Vitamin A combines with opsin, which is the red pigment in the retina to form rhodopsin (visual purple) when it is in form of retinol. Rhodopsin is necessary for visual adaptation to darkness. Retinol and retinoic acid, being other forms of Vitamin A, are essential for growth of bone, testicular and ovarian function, for embryonic development, and for regulation of growth and differentiation of epithelial tissues.

Vitamin B1 (thiamine) combines with ATP to form a coenzyme, thiamine pyrophosphate, which is necessary for carbohydrate metabolism.

Mode Of Action

Vitamin B3 or Niacin (after being converted to niacinamide) is a component of Nicotinamide Adenine Dinucleotide (NAD) and Nicotinamide Adenine Dinucleotide Phosphate (NADP). These co-enzymes are necessary for tissue respiration, glycogenolysis, and lipid, amino acid, protein, and purine metabolism. Niacin lowers serum cholesterol and triglyceride concentrations by inhibiting the formation of very low-density lipoproteins (VLDL). Niacin also causes direct peripheral vasodilation.

Side Effects Hepatotoxicity; cholestasis; hypersensitivity reactions.
Dosage 0.5gm daily given with multi-vitamin

Mode Of Action

Vitamin B6 (Pyridoxine) is converted (in erythrocytes) to pyridoxal phosphate and pyridoxamine phosphate. These act as coenzymes for various metabolic functions involving protein, carbohydrate, and lipid utilization. Its use as an antidote is due to the fact that it increases the excretion of certain drugs like cycloserine and isoniazid, which act as its antagonists.

Mode Of Action

Vitamin E is thought to act as an antioxidant as well as being a cofactor in some enzyme systems. It protects polyunsaturated fatty acids, in conjunction with dietary selenium, in membranes and other cellular structures from attack by free radicals. It also protects red blood cells against haemolysis.

Drug Indication Prophylaxis and treatment of Vitamin E deficiency, which does not occur except, rarely, in premature infants, when it causes haemolysis or in severe fat malabsorption.
Contra-Indications Sensitivity to vitamin E
Side Effects Side effects mostly at high doses. Blurred vision; dizziness; headache; diarrhea; nausea; stomach cramps; unusual tiredness or weakness.
Dosage Adult: Fibrosis-100 to 200I.U daily. Children under 1year: 50I.U daily, 1year and over: 100 I.U daily.

Mode Of Action

Vitamin K promotes the hepatic synthesis synthesis of clotting factors II, VII, IX, and X. It promotes synthesis of proteins necessary for normal calcification of bones.It is normally formed by bacteria in the intestine as well as being present in the diet hence its deficiency can arise when there is no bacteria in the gut e.g. in the newborn and in-patients on antibiotics. As it needs bile salts to be absorbed, its absorption is reduced in obstructive jaundice.

Drug Indication Prophylaxis and treatment haemorrhagic disease of the newborn (since vitamin K from the mother may possibly be insufficient due to its poor passage through the placenta and because intestinal bacteria responsible for its natural synthesis are not present for 1st 5-8 days after birth); in the management of overdose of anti-coagulants; in patients with fat mal-absorption (Vitamin K is fat soluble hence it is not absorbed in fat mal-absorption disorders); treatment and prophylaxis of various coagul
Contra-Indications Allergy to Vitamin K; liver function impairment. Menadiol may cause hepatotoxicity, hemolytic anemia, and kernicterus in children and is not recommended for use in newborns, especially premature infants. The risk of side effects is lower with phytonadione.
Side Effects Anaphylaxis; cyanosis; dizziness; transient hypotension; profuse sweating; rapid and weak pulse; hemolytic anemia and liver toxicity especially in children.
Dosage Should be based on laboratory tests of clotting functions. Dose 2.5 to 10mg, or up to 25mg.
Special Information All vitamin K compounds are 2-methyl-1,4-naphthoquinones. Two natural forms exist: vitamins K1 (phytonadione or phylloquinone) and K2 (menaquinone). Vitamin K1 is found in food. Intestinal bacteria synthesize Vitamin K2 found in human tissues. Menadiol sodium diphosphate (vitamin K4) is a water-soluble derivative converted in the body to menadione (vitamin K3).
